PLEASE READ AND CONFIRM PRIOR TO ATTENDING A CLINIC APPOINTMENT AT LEVEL 2
The following advice is important in order to keep yourself and others using the Health Hub facilities safe during this COVID-19 pandemic.
Please read ALL of the following statements, enter your name and click confirm if you are happy to comply with them. Thank you.
If you are unwell with any respiratory symptoms +/- a fever, please CALL US TO CANCEL your appointment and rebook when you are well again.
If you do not have any symptoms but in the last 14 days have had; close contact with a confirmed or probable COVID-19 case, travelled internationally, had direct contact with a person who has travelled overseas, worked in an international aircraft, shipping vessel or cleaned in any of these facilities, please CALL US before the day of your appointment to discuss appropriate PPE needs prior to your appointment.
Do not enter the Health Hub before your appointment. There will be no waiting room available inside the building. Your physiotherapist will come out to the front entrance to meet you and take you straight through to the clinic room at the time of your appointment.
Please use the hand sanitiser at the reception desk on entering and exiting the building.
We discourage friends or family members from accompanying you into the building but if it is necessary they will need to agree to comply with the same terms and conditions and join you in the clinic room for the duration of your appointment.
A QR code will be available at the front entrance to scan.
A face mask and gloves can be worn at your own discretion. They must be taken home with you at the end of the appointment. Your therapist will be wearing the appropriate PPE in compliance with the Ministry of Health guidelines.
Whilst in the clinic please try to follow the standard precautions stipulated by the Ministry of Health - Good hand hygiene, cough or sneeze into your elbow, use the hand sanitiser provided, maintain social distancing whenever possible.
